The Program Coordinator position at LifeScape is responsible to develop, coordinate, and monitor Individual Support Plans (ISP) for people receiving services with LifeScape. They will also advocate with, and on behalf of, the person supported in all respects, including individual choices and independence.
For this position, LifeScape is in search of an individual with excellent time management skills, in addition to knowledge of person-centered thinking tools. The ideal candidate will be able to work with a team, be flexible, and be able to think about the big picture for the individuals we support and LifeScape as a whole. The ability to be organized and have good oral/written skills would be a positive for this role.
Other roles and responsibilities for this role include, but are not limited to:
Maintain ongoing communication with participants, staff, guardians, parents, and Conflict-Free Case Managers about ISP plans, policies, concerns, etc.
Ensure that the person-centered plan is being implemented as written, and communicates with Conflict-Free Case Managers when the plan needs to be changed or updated
Provide oversight and maintenance of participants records to ensure that data is current and comprehensive
Participate in ISP meeting to assist in development of a comprehensive person-centered plan
Write the ISP Programs corresponding with assessment results and the Outcomes/Action plans of the ISP
Complete documentation as requested by Rep Pay Associate to support people to apply for/maintain SSA Benefits
Ensure compliance with all agency policies and procedures
Promote LifeScape's Mission and Core Values

Education and Experience
Must meet federal requirements to be Qualified Intellectual Disability Professional (QIDP), by having a bachelor's degree in a behavioral science, social science, education, or related field, or an equivalent combination of education and experience. One-year experience working with people with developmental or physical disabilities required; more than four years is preferred. A valid driver's license and clean driving record required. Successful completion and certification in training provided by agency within specific time allotment is required.
