The Program Coordinator is responsible for overseeing service delivery in compliance with State regulation, ensuring person-centered services, supervising direct support staff and team leads, coordinating client services, conducting required client visitations, and maintaining documentation standards. This role serves as the primary liaison between clients, support teams, case managers, and the organization to ensure quality, compliance, and continuity of care.The ideal candidate will possess strong management skills, experience in program development, and a passion for working with diverse populations with elderly and disabilities persons. Essential Duties & Responsibilities
Ensure all services are delivered in accordance with State policies and regulations
Monitor compliance with service plans, service agreements, and individual support plans
Ensure staff follow DHS-required procedures, including:
Rights restrictions
Incident reporting
Vulnerable adult protection
Emergency procedures
Prepare for and support DHS licensing reviews, audits, and corrective action plans
2. Client Coordination & Visitations
Conduct required client visitations as mandated based on service type and individual needs
Maintain regular contact with clients and/or legal representatives to assess satisfaction, safety, and service effectiveness
Ensure services are person-centered, culturally responsive, and aligned with the client's goals
Identify service gaps and coordinate corrective actions promptly
3. Staff Supervision & Support Team Leadership
Provide direct supervision to Support Team Leads and Direct Support Professionals (DSPs)
Ensure staff schedules meet service delivery requirements
Conduct regular supervision meetings, coaching, and performance evaluations
Support onboarding, training, and competency development for staff
Address staff performance concerns in accordance with HR and DHS requirements
4. Documentation & Record Management
Ensure accurate, timely, and compliant documentation, including:
Service delivery records
Progress notes
Client visit logs
Supervision notes
Incident reports
Review documentation for quality, accuracy, and compliance
Maintain confidentiality in accordance with HIPAA and DHS standards
5. Collaboration & Communication
Serve as a liaison between:
Clients and families
Case managers
Support team leads
Program leadership
Participate in care coordination meetings and service planning discussions
Communicate changes in client needs, risks, or service plans to appropriate parties
6. Risk Management & Quality Assurance
Monitor health and safety risks and ensure mitigation strategies are implemented
Respond promptly to incidents, emergencies, and service disruptions
Support continuous quality improvement initiatives
Ensure follow-up actions are completed and documented
Required Qualifications
Associate's or Bachelor's degree in Human Services, Social Work, Psychology, or related field (preferred)
Minimum
2 years of experience
working with individuals receiving 245D services
Experience supervising staff in a human services setting
Strong organizational, communication, and documentation skills
Ability to communicate effectively with staff, clients, families, and county partners
Preferred Qualifications
Experience serving culturally diverse communities
Leadership or program coordination experience
Familiarity with person-centered planning and trauma-informed care
Bilingual or multilingual abilities (preferred, not required)
Working Conditions
Combination of office-based and community-based work
Required travel for client visitations and supervision
Occasional evening or weekend availability as needed
Ability to respond to urgent client or staffing needs
