Job Summary: HOH, Inc. Community Supports Residential Programs are designed to enable persons receiving services to live as independently as possible in their homes. Coordinates and manages program operations while ensuring compliance with regulatory requirements.
Essential Job Functions:
1. Ensure that right to fair and equitable treatment self determination, individuality privacy, property and civil right, including the right to wage a complaint, are well established and maintained at all times for the people we support.
2. Coordinate program operations including attending medical appointments, coordinating activities, providing training and guidance to the people we support. Assist individuals with integration into the community by accompanying them to various recreational and social activities based on the interest of the individual. Provide transportation when necessary.
Ensure that personal hygiene and health standards are met for supported individuals.
Provide the person/people supported a meaningful learning experience to assist individuals in reaching their goals. Utilize least restrictive environments. Complete and oversee the collection of data when needed. Complete monthly summaries for all individuals. Make announced and unannounced visits as required by DDSD.
3. Monitor the safety and comfort of the people being supported, while ensuring living environments and program activities are in compliance with the regulatory requirements. Ensure that the fire and tornado evacuations are performed outlined in policy. Maintain and monitor the staff communication log. Ensure compliance with HIPAA in all program activities.
4. Supervises trains, evaluates, and initiates corrective action for Habilitation Training Specialists assigned to the Community Support Residential Programs. Accessible 24 hours/seven days a week and available to respond in person if necessary. Ensures HTS have completed DDSD mandated training and individualized training within specified timelines. Supervises activities, schedules, and prioritizes work in support of clerical and administrative needs of the program. Identifies administrative needs and develops appropriate solutions or recommendations. Develop and schedule staff meetings as necessary.
5. Notify Director or Manager when there is a change in a person's status. (i.e. medical, household problems, police issues, etc.) Documents and reports any accidents or client's rights violations, suspected abuse, neglect, or exploitation to the Director or The Compliance Officer as
appropriate. Monitor and document all records per program policy and procedures. Document according to the person-supported needs. Utilize approved behavior intervention techniques in appropriate situations.
6. Assist in arranging family visits. Assist all team members with the annual individual plan and any IP addendum. Act as an advocate in protection of their individual rights and responsibilities. Serve as liaison between community agencies in the service delivery system, families/guardians and the agency.

Required Qualifications and Skills:
Possess a minimum of four years of any combination of college level course work or full time equivalent experience in serving persons with disabilities, unless this requirement is waived in writing by the DDSD director or designee.
Experienced in using computer software such as: Microsoft Office products (Word, Excel, PowerPoint, and Outlook).
Demonstrated familiarity with DDSD/DHS policies and procedures.
Demonstrated supervisory experience.
Experience in Residential Support Services preferred.
Proficient communication skills.
Demonstrated experience in accounting, auditing, and budgeting skills.
Must possess and maintain a valid Oklahoma driver's license, dependable transportation, and automotive insurance.
Must be able to drive day or night anywhere.
