The Coordinator is responsible for the supervision of direct care staff and program services for multiple programs and ensuring that consumer needs are met, along with developing and implementing the Individualized Service Plan (ISP) for each assigned consumer based on the service specifications in the consumer's Comprehensive Plan of Care (CPOC) in accordance with established Agency policies and procedures, as well as state licensure requirements.
A bachelor's degree in a human services-related field with a minimum of one year experience working with the elderly and/or people with developmental disabilities, or a High School Diploma with an equivalent combination of three (3) years' education and experience working with the elderly and/or people with developmental disabilities. Good verbal, written, and interpersonal skills, the ability to represent the Agency in a highly professional manner, and to maintain the highest level of confidentiality. Good judgment, initiative, dependability, sound decision making skills, the ability to handle multiple responsibilities, strong organizational and time management skills, and the ability to prioritize work and meet multiple deadlines. A minimum two years of experience in managing staff and in the delivery of services to children and adults with developmental disabilities is required.
