Job Overview


We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will play a crucial role in supporting individuals and families by providing comprehensive case management services. This position requires a strong understanding of behavioral health, crisis management, and workforce development to effectively assist clients in navigating their challenges and achieving their goals.

Duties



Conduct assessments to identify client needs and develop individualized service plans. Provide ongoing support and guidance to clients, helping them access necessary resources and services. Collaborate with community organizations, service providers, and stakeholders to coordinate care for clients. Utilize motivational interviewing techniques to empower clients in their decision-making processes. Manage crisis situations effectively, ensuring the safety and well-being of clients. Maintain accurate records of client interactions, progress notes, and case documentation. Collaborates with the treatment team on a daily/weekly basis. Communicated with client and family on a weekly and as needed basis for all treatment decisions.

Qualifications



Bachelor's degree in Social Work, Psychology, or a related field is preferred; relevant experience may be considered in lieu of a degree. Experience in workforce development, behavioral health, or social work is highly desirable. Knowledge of crisis management techniques and best practices. Strong communication skills with the ability to build rapport with diverse populations. Proficient in motivational interviewing techniques to encourage client engagement. Ability to work independently as well as part of a multidisciplinary team. Commitment to maintaining confidentiality and adhering to ethical standards in case management practices.

Pay: $60,000.00 - $62,000.00 per year
