Collaborate with multidisciplinary teams on discharge planning, case management, and coordination of post-acute or hospice care services. Perform patient assessments focusing on anatomy, physiology, and medical terminology to develop tailored care plans for diverse populations including pediatrics, geriatrics, and individuals with developmental disabilities or dementia. Educate patients and their families on health coaching topics such as medication administration, nutrition, disease management (including Alzheimer's and infectious diseases), and wellness strategies. Ensure compliance with all safety protocols including infection control practices and proper use of medical imaging or specialized equipment like cardiac catheterization tools or neonatal ventilators.

Skills

Proven experience working with Electronic Medical Records (EMR) systems such as Epic, Cerner, eClinicalWorks, or Athenahealth. Strong clinical skills in critical care environments including ICU experience at Level I Trauma Centers or Level II Trauma Centers; hospital medicine; emergency medicine; or urgent care settings. Expertise in specialized areas such as long-term care (Dementia care, Memory care), hospice services, pediatric (PICU), neonatal (neonatal ventilator), or post-acute care environments. Proficiency in advanced nursing procedures including IV insertion/infusion therapy, catheterization (urinary/vascular), tube feeding management, sterile processing techniques, and suture placement. Knowledge of ICD coding (ICD-9/ICD-10) for accurate medical documentation and billing purposes. Ability to perform comprehensive physical examinations and patient assessments across various age groups using anatomy knowledge and physiology understanding. Familiarity with infection control standards and aseptic techniques to prevent healthcare-associated infections. Working knowledge of medical terminology related to cardiology (cardiac catheterization), ophthalmology (ophthalmology procedures), orthopedics (Hoyer lift use), or occupational health settings. Experience administering injections--including botulinum toxin treatments--and performing venipuncture with precision. Ability to work effectively in fast-paced environments such as emergency rooms or trauma centers while maintaining attention to detail under pressure.
