Part-Time
position with a schedule consisting of
10am-2pm, Monday-Friday, with a rotating on-call weekend.

Key Responsibilities
Provide
direct, hands-on nursing care
to patients of all ages in the home and facility environment.
Perform
comprehensive assessments
and identify patient and family needs.
Develop, update, and maintain individualized
nursing care plans
in collaboration with physicians and the care team.
Evaluate patient progress and promptly adjust care based on changes in condition or treatment orders.
Counsel patients and families on health and wellness topics, including nutrition, safety, and prevention.
Maintain accurate and timely documentation per agency and regulatory standards.

Qualifications
Graduate of an
accredited school of nursing
.
Current CT Registered Nurse (RN) license
in good standing.
Minimum
1 year of nursing experience
(hospital or acute care preferred).
Home Health or Private Duty experience
strongly preferred.
Valid driver's license, reliable transportation, and ability to travel throughout
Fairfield County, CT
.
Strong communication, documentation, and critical thinking skills.
