Licensed Practical Nurse Responsibilities:

Responsible for documenting and providing skilled nursing care to our patients, following the developed plan of care and the patient's physician orders for each of our patients; while adhering to confidentiality and professional boundaries. Ensure quality and safe delivery of home health care services by utilizing basic nursing skills and the nursing process. Recognize and institute when emergency measures are needed in a rapid, appropriate manner Participate in the coordination of home health services, appropriately porting the identified needs for other disciplines (i.e. PT, OT, ST) to the DON or RN supervisor

LPN Licensed Practical Nurse Qualifications

Valid license

Strong caregiving and patient care skills

Must be able to stand walk, squat, bend, kneel, reach, twist, push and pull. Varied assistance is required depending on the mobility of the client. Must be able to lift to 50 pounds

Knowledge of first aid and patient observation techniques

Participate in the coordination of home health services, appropriately reporting the identified needs for other disciplines (i.e. PT, OT, ST) to the DON or RN supervisor
