The Private Duty Registered Nurse (PD RN) provides one-on-one, in-home nursing care to patients who require continuous or intermittent healthcare services. The PD RN delivers comprehensive, individualized care for patients with chronic illnesses, disabilities, recovery from surgery, or complex medical conditions in a home setting.
We are looking for a nurse with open availability 5-7 days per week. You will be working with Medicaid patients privately in their homes. Work shifts are exactly 4 hours every day, 5-7 days per week.
Key Responsibilities:
Patient Care:
Ability to work with Tracheostomy care, feeding tube care, ventilators, total parental nutrition, severe seizures and wound VAC.
Administer prescribed medications and treatments, including injections, wound care, and intravenous (IV) therapy.
Monitor and document patient's vital signs, symptoms, and responses to treatments and medications.
Perform daily health assessments and adjust care plans accordingly.
Assist with personal care tasks as needed, such as bathing, dressing, and feeding.
Provide palliative or end-of-life care for patients as required.
Care Coordination:
Develop and implement patient care plans in collaboration with physicians, family members, and other healthcare professionals.
Serve as the primary point of contact for patient family members and caregivers.
Ensure smooth transitions between different levels of care or between hospital and home settings.
Patient Education:
Educate patients and families on managing health conditions, medications, diet, and general wellness.
Instruct families on caring for patients at home and provide support in understanding medical equipment or treatments.
Documentation:
Maintain accurate patient records, including assessment results, progress notes, and treatment plans.
Ensure documentation is up-to-date and compliant with legal and regulatory standards, including HIPAA.
Collaboration:
Work with interdisciplinary teams, including physical therapists, dietitians, and social workers, to optimize patient care.
Communicate regularly with physicians to update them on patient progress or concerns.
Compliance:
Adhere to state laws, agency policies, and industry standards regarding private duty nursing and patient care.
Maintain required certifications, continuing education, and comply with safety protocols.
Required Qualifications:
Licensure/Certifications:
Registered Nurse (RN) license in the state of Nevada.
BLS (Basic Life Support) certification.
Experience in Home Health or Private Duty Nursing.
Experience:
At least 1 yr of clinical nursing experience in home health, hospital or private duty nursing settings.
Experience with specialized care such as wound care, IV therapy, and post-surgical care is a plus.
Skills & Abilities:
Excellent critical thinking and assessment skills.
Strong interpersonal skills for interacting with patients, families and other healthcare professionals.
Ability to work independently and manage a caseload of patients.
Pay: $40.00 - $45.00 per hour
