Responsibilities
Assist residents or clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
Monitor patient health by observing vital signs such as blood pressure, pulse, and temperature; report any concerns promptly
Administer medications accurately following care plans and HIPAA regulations to ensure safety and compliance
Support residents with memory care needs, including Alzheimer's and dementia care strategies, ensuring a safe environment
Prepare nutritious meals tailored to individual dietary requirements and assist with feeding when necessary
Maintain cleanliness of living spaces by performing light housekeeping tasks such as cleaning bedrooms, bathrooms, and common areas
Use specialized equipment like Hoyer lifts or resident's to assist with transfers safely and effectively
Document observations, care provided, and changes in condition meticulously to ensure continuity of care
Follow established care plans created by healthcare professionals or social workers to meet each resident's unique needs
Engage residents in social activities or companionship to promote mental health and emotional well-being
Adhere strictly to safety protocols including first aid procedures and CPR certification when required
Support hospice or palliative care efforts by providing comfort measures and emotional support to patients and families
Requirements
Proven experience in home health, assisted living, nursing homes, or group homes is highly preferred
Knowledge of patient observation techniques and ability to monitor vital signs accurately
Experience working with people with developmental disabilities, autism spectrum disorder (ASD), Alzheimer's disease, or dementia is a plus
Familiarity with HIPAA regulations ensuring confidentiality of patient information
Ability to perform heavy lifting safely using equipment like Hoyer lifts or resident's when necessary
Certification in CPR and First Aid from recognized organizations is required; additional certifications such as hospice or memory care training are advantageous
Strong communication skills with the ability to work collaboratively within a multidisciplinary team
Compassionate attitude with patience, empathy, and respect for residents' dignity and independence
Flexibility to work various shifts including evenings, weekends, or overnight shifts as needed
