Mulberry Health is a clinician-owned mobile medical group providing in-home primary care, transitional care, and palliative care to patients across Arizona. We serve seniors in private homes, assisted living, and memory care communities--bringing compassionate care directly to where it's needed most.
Our physician-led model ensures patients are seen quickly, efficiently, and with continuity of care. We are expanding our primary care division and seeking a dedicated Nurse Practitioner or Physicians Assistant to help us deliver comprehensive, home-based care to patients across the Phoenix metro area.
We are seeking a Primary Care Nurse Practitioner or PA to provide in-home visits for patients with chronic conditions, post-acute needs, and general primary care requirements. You'll serve as the patient's primary care provider, managing their health holistically and building long-term relationships.
Responsibilities
Conduct in-home primary care visits across the Phoenix metro area (This role's territory will be Phoenix/West Valley area)
Manage chronic conditions and preventive care
Coordinate care with specialists, caregivers, and family members
Document visits using eClinicalWorks EHR
Educate patients and caregivers on health maintenance and disease prevention
Participate in care planning and interdisciplinary team meetings
Qualifications
Certified Nurse Practitioner (FNP-C, AGNP-C, or equivalent) or Physicians Assistant
Active Arizona license
Experience in home health, assisted living, or primary care preferred
Strong communication and clinical documentation skills
Comfortable with mobile care and telehealth platforms
Ability to work independently and manage a flexible schedule
