Job Summary


We are a newly revamped, long standing primary care practice that is seeking a dedicated and compassionate Primary Care Nurse Practitioner, with good energy to join our healthcare team. The ideal candidate will provide comprehensive patient care, focusing on preventive health, chronic disease management, and acute care. You will work collaboratively with physicians and other healthcare professionals to ensure the highest quality of care for our patients across various settings, including outpatient clinics, assisted living facilities, and home care environments.

Duties



Conduct thorough patient assessments, including physical examinations and medical history reviews. Develop and implement individualized care plans based on patient needs and preferences. Perform diagnostic evaluations and interpret results to guide treatment decisions. Administer medications, injections, IV infusions, and other treatments as required. Provide education to patients and families regarding health management, disease prevention, and wellness strategies. Collaborate with interdisciplinary teams to coordinate care for patients with complex medical conditions. Maintain accurate medical records and documentation in compliance with HIPAA regulations. Participate in clinical research initiatives as needed to enhance patient outcomes. Manage acute pain through appropriate interventions and referrals when necessary. Provide triage services for patients presenting with urgent health concerns.

Requirements



Master's degree in Nursing or a related field with a valid Nurse Practitioner license. Proficiency in EMR/EHR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks. Experience in primary care settings; familiarity with outpatient services is preferred. Demonstrated ability in various clinical skills including suturing, IV insertion, phlebotomy, and diagnostic evaluations. Knowledge of ICD-9/ICD-10 coding and CPT coding for accurate medical documentation. Experience in geriatrics, pediatrics, or working with individuals with developmental disabilities is a plus. Familiarity with specialized areas such as gastroenterology, infectious disease care, behavioral health, or hospice & palliative medicine is advantageous. Strong communication skills to effectively educate patients and collaborate with healthcare teams. Ability to perform procedures such as catheterization, spinal taps, or airway management when necessary. Commitment to ongoing professional development and adherence to NCQA standards. Pay: $75,000.00 - $115,000.00 per year
