SM This Pre-Visit Care Connector position is responsible for coordination of care prior to a HouseCalls Visit to ensure the documentation of accurate and up to date information. Care Connectors also assist with communication to Providers (both internal and external), as well as members and their families during and after their HouseCalls visit. The HouseCalls Care Connectors will work in a call center environment virtually from their own home to provide immediate care coordination needs via telephonic incoming calls. This position is full-time. Employees are required to have flexibility to work any of our 8-hour shift schedules during our normal business hours of 8:00am to 8:00pm CST. We offer approximately 3 weeks of training. The hours during training will be 9:30am to 5:30pm.
