The Practice Manager II is a valuable member to our Health Care Operations team who is a key operational position to achieve Lake County Health Department and Community Health Center's (LCHD/CHC) mission to promote the health and well-being of all who live, work, and play in Lake County. The Practice Manager oversees all operations of their assigned Health Center, while fostering strategic system-wide collaboration to assure optimal client safety, satisfaction, and health outcomes. The Practice Manager will work in a dyad with the Health Center Medical Director to focus on clinical operations, improvements of quality measures, and strategic planning for the health center. This leader will work closely with their peers and senior leadership to assure their team is fully competent and supported to provide quality care. The Practice Manager will ensure all aspects of health center operations run smoothly and provide direct supervision to the Office Manager and the Nurse Manager. The Practice Manager/Health Center Medical Director dyad will ensure their health center is following the LCHD/CHC operational standardizations set forth. Additionally, the Practice Manager will actively engage in outreach activities with the community and organizations in the geographic service area of their health center.

Essential Functions

Provides overall management of daily operations of the Health Center.
Provides management direction of daily operations of the Women's Health and Family Planning Programs
Ensures the Women's Health and Family Planning programs are continually operating in compliance with HRSA, TJC, and Title X standards.
Identifies and actively seeks opportunities for improvement based on uniform data system (UDS) measures and key performance indicators (KPI) and other relevant data to improve client outcomes, satisfaction, and health center performance.
Maintains current knowledge of NextGen EHR/EPM and Tableau.
Leads continuous quality improvement (CQI) initiatives at the health center in accordance with the Quality Management System (QMS).
Recognizes those who contribute to quality, efficiency and improvement in operations, programs and services.
Works in a dyad relationship with the Health Center Medical Director, including weekly meetings to review current quality, clinical operations and access metrics and identify successes and opportunities for improvements.
Manages aspects of people management for Nurse Manager and Office Manager, including staffing, hiring, onboarding, discipline, payroll, safety, facilitating monthly one-on-one meetings, team meetings, communications, training, and annual performance evaluations.
Assures ongoing compliance with agency policies and guidelines, accreditation, and regulatory standards including but not limited to LCHD/CHC, Joint Commission, Health Resource and Services Administration, Clinical Laboratory Improvement Amendments, dental equipment sterilization and other measures of health center infection prevention.
Monitors staff and client scheduling and adjusts accordingly to ensure the highest level of care per the Patient Centered Medical Home (PCMH) care model.
Develops, monitors, and analyzes health center budget in accordance with County and agency guidelines and in conjunction with the Business Manager, ensures health center operations are meeting projected revenue and expenditure goals.
Monitors and maintains supply inventory and oversees ordering/purchasing activities in collaboration with Nurse Manager and Office Manager per established policies and procedures.
Resolves client complaints either in person or over the phone as necessary.
Collaborates with senior leadership to develop department priorities, goals, and maintain standardization through a systems approach.
Attends meetings as required and participates in committees as directed.
Serves as primary contact for facility related issues, such as for maintenance and Information Technology (IT) needs.
Maintains required training, licensure, and/or certification.
Maintains confidentiality of privileged information and adheres to client privacy laws.
Demonstrates culturally competent healthcare delivery and fosters same among all staff.
Maintains a positive, supportive environment conducive to attaining high staff morale and job satisfaction.
All other relevant duties as assigned.
Bachelor's degree preferable in a healthcare field.
At least 5 years of direct patient-facing experience in an ambulatory/general medicine setting.
LPN, RN, APRN, PA, RDN, LDN, LCSW or comparable clinical title required.
Two years of management experience preferred.
For internal candidates - education requirements may be offset by performance, skills and leadership demonstrated during time as an LCHD employee.
