Job Summary

• The Population Health Support Partner is accountable for an integral role on Avera\xe2\x80\x99s population health team in the ingestion, analysis, and dissemination of reports related to financial, care management, and network metrics of Avera\xe2\x80\x99s value-based initiatives. These reports will inform clinical and operational decision making.
• The individual role requires expert knowledge on value-based strategies including attribution and quality metrics.
• This individual will assist the department leadership in the day-to-day management of Avera\xe2\x80\x99s value-based initiatives, strategic direction, and planning.

Education and/or Experience

• Bachelor\xe2\x80\x99s degree required.
• Experience related to healthcare performance improvement processes is preferred.
• Basic knowledge of accountable care organization attribution and quality metric is preferred.

Licensure/Certification/Registration

• This position requires a valid driver\xe2\x80\x99s license and that the employee is insurable by the Avera Health automobile liability insurance carrier.

Work Schedule

• Monday-Friday, 8:00am-5:00pm; 80 hours every 2 weeks

Pay

The hourly range for this position is listed below. Actual hourly rate dependent upon experience.

• $21.50 - $32.50 per hour
