FUNCTIONAL OR SCOPE OF ASSIGNED DUTIES
Clinical (approximately 95%):

In addition to compliance with national and local regulatory, policy, and accreditation requirements governing this health care system, this position encompasses the following responsibilities, in partnership with his/her section chiefs, and supervisors. The incumbent requires appropiate time mapping and resources to ensure they are empowered to meet expected proficiencies in administration, clinical practice, education, and research. * Primary focus is clinical care coordination for patients in the FVAHCS Transplant Program.
  • Assists with the coordination of transplant care, in collaboration with FVAHCS Medicine Service Transplant Coordinators and the Community Care Transplant Coordinator RN, for patients who are either: undergoing transplant work-up, currently listed for transplant, or are status posttransplant.
  • Participates as a member of the multidisciplinary team to facilitate transplant services for Veterans throughout the FVAHCS.
  • Facilitates patient\'s entry and exit from various health care systems by establishing and maintaining liaison with a variety of medical specialists and associated staff within this facility, other VA facilities, VA Transplant Centers and non-VA transplant facilities.
  • Collaborates and coordinates with other health care resources to ensure continuity of care.
  • Keeps abreast of the latest VHA Transplant Program policies and procedures and serves as a resource person for medical center staff.
  • Follows the Office of Community Care policies and procedures for transplant referrals sent to FVAHCS Community Partners.
  • Utilize the Transplant Referral and Cost Evaluation/Reimbursement (TRACER) system for initial review of transplant candidates.
  • Monitor and capture transplant related care and events for Veterans completion of clinical requirements for TRACER submissions and Community Care referrals.
  • Review and process consults for other categories of care other than transplant
2. Clinical Duties: The incumbent is expected to remain privileged and practicing in medical services. The exact privileges and procedures for a particular provider will be approved by the Professional Standards Board. All practitioners must meet the following qualifications:
2.1. Patient Care: Practitioners are expected to provide patient care that is compassionate, appropriate, and effective for the promotion of health, prevention of illness, treatment of disease, and care at the end of life. Tasks also include indirect patient care: open encounters and open consult maintenance, CPRS documentation requirements, and monitoring of clinic management.
2.2. Medical/Clinical Knowledge: Practitioners are expected to demonstrate knowledge of established and evolving biomedical, clinical, and social sciences and the application of their knowledge to patient care and the education of others.
2.3. Clinical Judgment: To ensure practice-based learning and improvement, practitioners are expected to be able to use scientific evidence and methods to investigate, evaluate, and improve patient care.
2.4. Interpersonal and Communication Skills: Practitioners are expected to demonstrate interpersonal and communication skills that enable them to establish and maintain professional relationships with patients, families, and other members of the health care team.
2.5. Professionalism: Practitioners are expected to demonstrate behaviors that reflect a
commitment to continuous professional development, ethical practice,and understanding and sensitivity to diversity, and a responsible attitude toward their patients and their profession.

Administrative (approximately 0-5%): * FVAHCS Transplant Program liaison to the National Transplant Program Office, VISN Office, FVAHCS Executive Leadership Team, FVAHCS Service Chiefs, and Service Line Transplant Coordinators regarding all FVAHCS transplant issues.
  • Creates and maintains a database to track all FVAHCS transplant cases, following the Veterans from initial evaluation, TRACER submission, in conjunction with the Community Care Transplant Coordinator.
Education/Research (approximately : * Promotes and participates in the delivery of patient and family transplant education related to the coordination of transplant services and community care services.
  • Participates, directs, and provides opportunities for staff development through in-service education as needed.
  • Assures self-development responsibilities and maintains current knowledge of VHA transplant services and Office of Community Care services.

Physician Assistant Basic Requirements
  • Master\'s degree in any discipline or a bachelor\'s degree in any discipline with one year of experience as a Physician Assistant ~AND~
  • Successfully passed the PA National Certifying Exam (PANCE) (or are you in the process to taking the exam) ~AND~
  • Currently hold, or will hold, a full and unrestricted license to practice in a State, Territory, or Commonwealth of the United States or in the District of Columbia.
(1) Physician Assistant-I (PA-I)(a) Experience and Education. None beyond the basic requirements. This is the entry level for PAs.(b) Demonstrated Knowledge, Skills, and Abilities. The following list of knowledge, skills, and abilities (KSA) present the minimum skill set expected of an Entry Level PA. This practice may include but is not limited to:i. Ability to utilize the principles of PA practice in the care of assigned patients to provide appropriate care.ii. Ability to perform patient history and physical.iii. Ability to formulate a plan and provide treatment to the patient.iv. Ability to provide patient care that is abreast of changing concepts and advancements in the profession.
