Position is fully remote. Franciscan Health provides equipment. Worker must be agreeable to Franciscan Health Flexible Work Agreement.*


Flexible availability: including days, evenings, and weekends


Minimum of 12 visits/month required.

WHAT YOU CAN EXPECT

Pelvic Health experience preferred

Establish a plan of care that is realistic and has measurable goals. Communicate and involve other disciplines, physician, family and patient in developing care plan. Provide those services or treatments requiring physical therapist skill according to the written patient care plan and the signed plan of treatment. Demonstrate understanding of correlation between OASIS assessments and outcomes reporting. Communicate regularly with all disciplines involved in the care of the patient. Complete and initial and ongoing assessment (OASIS if appropriate) to determine level of functioning. 1:1 Therapist/Patient Focused Care Mentorship PRN Days Opportunity to work at various Franciscan Health therapy locations.

QUALIFICATIONS

Minimum of Bachelor of Science Degree from an accredited program is required Must have a valid license or temporary permit for the applicable state Experienced and new graduate Physical Therapist eligible Adapt and respond quickly to changing priorities Ability and willingness to work in team environment
