What You'll Do

Provide 1:1 outpatient physical therapy in clients' homes under PT supervision. Deliver functional, personalized, evidence-based treatments. Educate clients and caregivers on safe movement and mobility strategies. Collaborate with the team to create effective, client-centered care plans.

What We're Looking For

Licensed

Physical Therapist Assistant (PTA)

in Connecticut.

3+ years of experience

(SNF, traditional home health, or outpatient preferred). Experience or interest in

neuro rehab / Parkinson's Disease

a plus. Strong communication skills, clinical independence, and passion for helping older adults thrive.

Pay: $36.00 - $45.00 per hour
