Job Description

Touchstone Health, a full-service home health and hospice agency, is seeking a qualified and compassionate Physical Therapist Assistant (PTA) to care for our patients. Under the supervision of the Physical Therapist and in accordance with the interdisciplinary plan of care, our PTA\'s assist patients with following their individual programs of care to increase functional ability and reestablish independence at home. Travel the San Antonio area-North and Far North San Antonio

Duties & Responsibilities:

• Performing home visits to evaluate patient\'s level of function
• Assist patients with following the treatment program in their home setting
• Reassessing clinical signs and symptoms to determine effectiveness of program
• Reporting changes in the patients level of function to supervising Physical Therapist and collaborating to improve the treatment plan
• Instructing patient, family, and interdisciplinary staff regarding participation in the treatment program

• Degree in physical therapy from a university accredited by the American Physical Therapy Association
• License to Practice by the State of Texas
• Comprehensive knowledge of the principles, concepts, and methodology required of an physical therapist and its practice application in the treatment of hospice patients at home
• Excellent Communication and documentation skills
• Willingness to work with a interdisciplinary team
• Valid driver\xe2\x80\x99s license and reliable, insured transportation
