As a

Physiatry Nurse Practitioner

, you will partner closely with a supervising Physiatrist and the interdisciplinary care team to manage the medical and rehabilitative needs of patients recovering from surgeries, neurological events, or other function-limiting conditions. You'll play a key role in improving outcomes by facilitating smooth recoveries, reducing re-hospitalizations, and promoting independence.

Job Requirements:



Active and unrestricted Nurse Practitioner license in the state of Illinois Board-certified as an Adult, Geriatric, or Family Nurse Practitioner Master's Degree in Nursing from an accredited program Experience in PM&R, geriatrics, internal medicine, or SNF settings preferred Strong clinical skills in functional assessments, pain management, and rehab-focused medical care Comfortable working both independently and in collaboration with a supervising physician EMR proficiency and commitment to timely charting and care coordination

Key Responsibilities:




As the

Physiatry Nurse Practitioner

, you will serve as a functional medicine specialist in the facility:

Performing comprehensive assessments focused on rehab goals, mobility, and pain Developing and updating individualized treatment plans alongside the supervising Physiatrist Conducting regular follow-ups to monitor progress and modify plans of care Participating in interdisciplinary care team meetings and patient rounds Collaborating with therapy teams to track rehabilitation milestones and address barriers to progress Educating patients and families about diagnoses, recovery goals, and functional strategies Documenting visits, interventions, and outcomes clearly and promptly in the EMR Supporting discharge planning and care transitions to ensure continuity of rehabilitation progress Serving as a clinical resource to staff for musculoskeletal and neurologic conditions

Salary & Compensation:



Annual Pay:

$105,000 - $130,000 based on experience
