The PACE Personal Care Attendant II provides psychological, physical and emotional support and answers questions as appropriate to participant needs.

WHAT YOU CAN EXPECT

Practice Hours: Monday - Friday, 8:00 a.m. - 5:00 p.m. No Weekends, Evenings, or Holidays Performs a variety of care activities in a variety of care settings, after appropriate training, including and not limited to: providing psychological, physical and emotional support to participant and family; applying anti-embolism stockings; utilizing Med Sled; Emptying ostomy pouch, spot checking Oximeter with flow documentation; collecting specimens of stool and urine; Urinary catheter care; Completing ADLs and showering. Assists with clinic duties, daily care, transporting, ambulation, and care in the home of participant as assigned by the nursing staff such as- light housekeeping, driving and escorting and transportation in company vehicle to external appointments or grocery shopping, ADL care and showering, specimen collection and companionship. Uses medical equipment to physically assist participant transfers and mobility, not limited to power wheelchairs, manual wheelchairs, walking aids, oxygen and others. Answers questions about the treatment plan within the expected scope or refers questions to nursing staff. Answers questions related to daily activities to ensure that participants are comfortable and that they understand all the information provided to them. Takes vital signs, and document and records activities of daily living, intake, and output as assigned by the Nursing staff. Communicates and reports participant data and information to the clinic to ensure appropriate and timely care is provided to the participant. Reports any participant concerns, service requests, and safety observations to appropriate IDT members. Escalates important information to proper staff regarding any changes, progress, or unusual events concerning any participant. Maintains proficiency in point of care test protocols and procedures, including properly labeled specimens. May collect, analyze and record all required demographic, insurance/financial, and clinical data necessary to verify and record participant information. Documents all care encounters in the Electronic Medical Record (EMR) in a timely and accurate manner. Organizes and makes changes to the physical environment to ensure the environment is equipped to meet participant needs. Responds to participants and directs the inquiry to the appropriate party. Responsible for documenting service requests and informing appropriate IDT member(s) of service requests as required by PACE regulations. May serve as The PACE PCA designated representative of the PACE IDT to meet CMS regulations. As a PACE IDT member, the PACE PCA takes part in shared decision making, authorization, and care responsibilities to provide required services. Works with and supports participants with meals, fluids, and nourishment. Participates in monitoring, identifying and reporting safety-related issues. Maintains clinic supplies, prepares exam rooms and medical records, enters charges, and answers a multi-line telephone to assist in the safe and efficient operation of the clinic.

QUALIFICATIONS

High School Diploma/GED- Required 1 Year- Prior experience working with frail/elderly population- Required 1 Year- Healthcare experience as a patient care technician or similar position- Preferred Basic Life Support Program (BLS) - American Heart Association- Required Certified Nursing Assistant (CNA)-

State of Indiana

Licensing Board - Required

OR*
Health Unit Coordinator (HUC) - Required

OR*
Franciscan Patient Care Assistant Program (FPCAP) - Required
