Job Description

Minn Care Home Health is looking for PCA staff for a client in the St Paul area:


Full Time/Part Time Positions Starting at $16.00
Minn Care Home Health provides home care services to adults with developmental and mental disabilities, and those in need of medical care after hospital discharge. The PCA will perform direct client cares ordered by the client's physician and as documented in the client's care plan.




MAIN RESPONSIBILITIES:





1. Assist clients with personal care: dressing, grooming, bathing, eating, toileting, transferring, positioning, mobility.



2. Housekeeping: cleaning all living areas, making the bed, dishes, laundry, taking out the garbage.



3. Monitor and re-direct client behaviors to ensure their safety.



4. Meal planning and preparation.



5. Medications: reminders



6. Accompany clients to appointments.



7. Assist clients in getting around and participating in the community.



8. Complete daily progress notes at the end of each shift.



9. Report all incidents, complaints, injuries, health concerns, accidents, illness, change in schedule, verbal/physical altercation involving client, communication with clients' health providers to human resources and management staff.




REQUIREMENTS





A. Must pass the Department of Human Services background check



B. Must be reliable and have a strong work ethic



C. Must be fluent in English and have good communication skills



D. Must show compassion and empathy to clients



E. Must have a driver's license
