Responsibilities
Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, toileting, and mobility support
Monitor patients' health by taking vital signs and observing for any changes in condition or behavior
Support medication administration following proper protocols and HIPAA regulations to ensure privacy and safety
Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary
Maintain detailed care plans to track progress and update care strategies in collaboration with healthcare teams
Use specialized equipment such as Hoyer lifts or resident's to ensure safe transfers and mobility assistance
Conduct patient observation to identify any health concerns or behavioral changes promptly
Perform light cleaning duties to maintain a safe and hygienic environment in clients' living spaces
Document all care activities accurately, adhering to confidentiality standards like HIPAA
Provide emotional support and companionship to promote mental well-being and reduce feelings of isolation
Collaborate with nurses, social workers, and other healthcare professionals to deliver comprehensive patient care
Participate in training sessions on CPR, first aid, dementia care, Alzheimer's care, memory care, hospice care, autism support, and other specialized areas as needed
Qualifications
Prior experience working in assisted living, home health, nursing homes, or group homes is highly desirable
Knowledge of patient observation techniques and ability to monitor vital signs effectively
Certification in CPR and first aid is preferred; ongoing training opportunities are available for skill enhancement
Experience working with people with developmental disabilities, autism, dementia or Alzheimer's is advantageous
Ability to lift heavy objects safely and assist with transfers using equipment such as Hoyer lifts or resident's
Familiarity with medication administration procedures following HIPAA guidelines
Strong communication skills with an empathetic approach toward clients' needs
Ability to work independently as well as part of a multidisciplinary team
Flexibility to work various shifts including evenings, weekends or holidays as required
