We are seeking a dedicated and compassionate

PER DIEM Field Nurse for CERRITOS, CA to join our team who is predominantly fluent in speaking Spanish and English. In this role, you will provide high-quality nursing care to patients in various settings, including their homes and community environments. Your expertise will help ensure that patients receive the necessary medical attention and support for their health needs. The ideal candidate will possess a strong clinical background and be skilled in patient assessment and care management.

Responsibilities

Conduct comprehensive patient assessments to determine health needs and develop individualized care plans. Provide nursing care in home and community settings, ensuring adherence to infection control protocols. Assist with family planning services, offering education and support as needed. Perform critical care transport for patients requiring immediate medical attention. Administer treatments, including dermal fillers and dialysis, as per physician orders. Collaborate with multidisciplinary teams to coordinate patient care and follow-up services. Maintain accurate patient records using eClinicalWorks and other electronic health record systems. Educate patients and families on health management, medication administration, and lifestyle modifications. Ensure compliance with all relevant healthcare regulations and standards.

Skills

Proficiency in patient assessment techniques and critical thinking skills. Experience in home community care settings is highly desirable. Knowledge of infection control practices to safeguard patient health. Familiarity with internal medicine principles is a plus. Ability to lift patients safely and assist with mobility as required. Experience with critical care transport procedures is beneficial. Strong communication skills to effectively interact with patients, families, and healthcare professionals. Ability to work independently in diverse environments while maintaining high standards of care.
