GENERAL JOB DESCRIPTION



Reporting to the Executive Director, the Case Manager-Mentor has responsibility assisting Generation Diamond program participants with navigating community resources and services as they reestablish their life in the community. The Case Manager-Mentor administers assigning, monitoring, and organizing the program of Generation Diamond to best serve its program participants. The Case Manager-Mentor will be directly involved in the lives of their clients and provide resources and assistance. Developing short-, medium- and long-term life plans with each Generation Diamond participant is crucial to their success. These plans include job readiness or employment, working with helping professionals including social workers and doctors, and scheduling treatment or recovery as needed. The Case Manager-Mentor helps participants to adjust those plans as needed to best face their circumstances. The Case Manager-Mentor also follows up on each participant to ensure that they are reintegrated in the community with sound mental and physical health, along with stable housing and employment.

MAJOR DUTIES AND RESPONSIBILITIES



The Case Manager-Mentor assists and assesses Generation Diamond participants through these high-communication activities:

Screening. Working in close relationship with the Intake Coordinator, the Case Manager-Mentor makes the crucial evaluation of the suitability of each applicant to enter the Generation Diamond program. Assessing. Working in close relationship with the Intake Coordinator, the Case Manager-Mentor makes a general assessment of the applicant's health, emotional stability, and evident challenges (for example, addiction, a physical complaint, etc.) to develop a baseline entry evaluation. Risk evaluation. The Case Manager-Mentor assesses and reviews the risk factors attached to each applicant, assessing their possible/likely impact (positive or negative) on the applicant, the applicant's family or friends, a potential employer, Generation Diamond staff, even the public at large. Risk evaluation is a primary category to consider in determining whether an applicant is received into the Generation Diamond program. Planning. The Case Manager-Mentor develops short-term, medium-term, and long-term life programs with each participant. These programs will focus on positive action that can be taken by the applicant to decisively move toward a life purpose, a steady job paying livable wages, and stable housing. Implementation. The Case Manager-Mentor will guide, coach, and encourage program participant to take responsibility for and actively participate in their reintegration into the community. In partnership with program participants, the Case Manager-Mentor works with participants as they take practical action to implement their life plans toward steady employment and stable housing. They will evaluate and adjust implementation as needed to succeed. Crisis Response. The Case Manager-Mentor provides assistance and support to program participants in crisis situations, when needed. Outcome evaluation. Outcomes are evaluated at every turn in the Generation Diamond program, including the evaluations that assess long-term results. The goal is for participants to still be employed and housed after the program with checkups by their Case Manager-Mentor after the program at intervals of three months, six months, one year, and two years. The above require detailed and current record keeping, involving creating and maintaining an Excel database file to track participant progress in the Generation Diamond program.

GENERAL DUTIES AND RESPONSIBILITIES



Attend staff meetings, individual and/or group supervision, other meetings as necessary. Participate in opportunities to educate and advocate for increased awareness of the issues surrounding reintegration best practices. Contribute to a collaborative work environment assisting team to best serve the mission of Generation Diamond. Upon request or assignment, secure the building at closing time. On occasion, sub for the receptionist to receive guests or deliveries. Upon assignment from Executive Director, assist colleagues.

SKILLS, QUALIFICATIONS AND ATTRIBUTES



The Case Manager-Mentor must demonstrate a passion for and willingness to support Generation Diamond's mission, vision, values, and work and possess the following:



Associate or bachelor's degree in social work or related field; MSW or related graduate degree preferred. Minimum two years' experience in social service or related work. Certain internships may fulfill this requirement. Bilingual in Spanish and English preferred; or willingness to learn. Inclusive, empathetic worldview and demonstrated people skills to work with a variety of people every day. Knowledge and/or experience of jail/prison environment and knowledge of the criminal justice system preferred. Familiarity with re-entry and/or homeless community desired but not required. Listening and interview skills are crucial. Demonstrated competence in computer skills and database/file management. Positive attitude toward colleagues, participants, and the public. Willingness to serve citizens re-entering the community after incarceration, as well as their family members. Driver's license preferred.

WORK ENVIRONMENT



The work takes place in a general office atmosphere. Most duties are performed while sitting at a desk, table, or workstation. Required use of computers. May involve frequent, moderate lifting of objects up to twenty-five (25) pounds. Job estimated physical requirements: Occasionally lift 15 lbs.; walking 10%, standing 30%, sitting 60%. Smoke-free environment. This position will have direct contact with program participants with potentially violent or abusive individuals.

HOURS - 25 Hours per week
