Job Description

We are hiring for a Peer Support Specialist (PSS) to support One Community Health's health related social needs (HSRN) case management. Under the supervision of the Community Health Program Manager, the PSS will function as a role model to peers; exhibiting competency in personal recovery and use of coping skills; serve as a consumer advocate, providing patient information and peer support for OCH patients. The PSS performs a wide range of tasks to assist peers in the model of recovery within the community, providing outreach, building community, assisting with systems navigation, while promoting recovery and resiliency.


Qualifications:




Knowledge of the Recovery process and the ability to facilitate recovery within the integrated care setting. Knowledge and skills sufficient for establishing effective relationships with peers in recovery to support them in their own recovery. Knowledge of the signs and symptoms of mental illness (i.e. auditory and visual hallucinations, aggressive talk and behavior, thoughts of self-harm or harm towards others, isolation) and the ability to assist the patient to address symptoms using strategies such as positive self-talk. Knowledge of how to establish and sustain self-help (mutual support) and educational groups by soliciting input from patients on their strengths and interests. Have a history of receiving behavioral health services for a substance use disorder and be well-grounded in one's own recovery, with at least a year since initial diagnosis preferred.

Skills:




Function well in an interdisciplinary team approach to treatment through collaborating on patient care. Desire to deliver high quality services to a diverse multicultural community. Willingness to learn new skills and methods of behavioral health care delivery. Successful relationship management skills involving interdisciplinary and multicultural teams and maintain a high degree of professionalism. Knowledge of and compliance with HIPAA and CFR 42 Part 2 confidentiality regulations. Ability and willingness to work in a dynamic and changing community health care environment

Education/Certification/License:




High School graduate or equivalent. Eligible to become certified as a Peer Support Specialist on the State registry

available.
Current CPR/BLS Certification in accordance with OCH Policy Successful completion of credentialing and privileging in accordance with OCH policy.

Compensation:




Exempt: starting at $23 hourly, depending on experience. Bilingual Differential: Additionally, we offer $0.75 per hour as compensation for bilingual proficiency in both English and Spanish. Milage Reimbursement: In accordance with OCH policy.
