CMHA is looking to hire a Peer Navigator for our Young Adult Assertive Comm Treatment (YAACT) program. . This position will be located at 29 Russell St. New Britain CT 06051. Monday - Friday.

Compensation Range:

The salary range for this position starts at $18.00 per hour.
Compensation for each position is determined not only by years of relevant experience, education, and skills, but also by maintaining internal pay equity. CMHA also takes into consideration our benefits and paid time off package to provide a well-rounded and competitive approach to the applicant's overall compensation.

Position Summary:

The Peer Navigator provides initial and ongoing outreach and support to potential and current clients. They support, teach, advocate and empower individuals with behavioral health and Substance use disorders; which includes developing person centered recovery goals. Liaison with local emergency rooms, detox centers and treatment facilities. They engage in traditional and recovery oriented systems navigation, coordination of services, skill building and eliminating any barriers to care. The goal is to empower and educate clients to rediscover their ability to pursue and achieve a higher quality of life that allows them to prosper in the community.

Essential Responsibilities:


Builds trusting, recovery orientated rapport with individuals with mental health & co-occurring disorders and can assist with linking to team. Provides input and advocacy by actively participating in clinical team meetings. Selectively utilizes personal recovery experience to provide hope and empowerment to assist & motivate. Demonstrate and role model effective skills to lead towards recovery. Engages clients within clinic and community to assist with navigation throughout the agency. Facilitates client groups based on topics relevant to the population. Produces concise documentation regarding on-ongoing services with client; including progress and recommendations. Work with family members by providing education and support as needed/if applicable. Works with client to achieve and maintain skills to produce an independent and self-sustaining life; including assistance with finances and entitlements, support with healthy activities of daily living skills and supervision of medications Performs other related duties as assigned.

Requirements:


High school diploma or equivalent Completion of a Peer Recovery training program Personal experience with substance abuse recovery Strong communication and interpersonal skills Ability to work effectively in a team setting Knowledge of community resources and support services
