to provide specialized in-home services under the Children's Waiver (HCBS). Our mission is to support children and youth living with chronic medical conditions or life-threatening illnesses by helping them manage pain, relieve symptoms, and preserve quality of life. This service plays a vital role in reducing suffering, increasing comfort, and empowering families through clear guidance and clinical support.
As part of PSCMH's palliative care team, you will work directly with medically fragile children and their caregivers to ensure safe symptom monitoring, effective pain management strategies, and individualized care tailored to the child's specific condition.
Responsibilities:
Teach participants how to use prescribed pain medications safely and effectively
Monitor symptoms, identify early warning signs, and help families recognize patterns of worsening or improving symptoms
Support caregivers in understanding the child's symptoms, responding safely, and knowing when additional medical assistance is needed
Provide individualized, in-person clinical guidance to manage pain, discomfort, and symptom progression
Deliver goal-oriented care outlined in the HCBS Service Plan and Plan of Care
Collaborate closely with the child's healthcare team and Care Manager
Modify service frequency and scope as the child's medical needs evolve
Complete timely, accurate, and thorough documentation
Adhere to PSCMH's compliance expectations
for Children's HCBS, including Medicaid documentation, service delivery standards, and recordkeeping requirements
Service Delivery:
Services are delivered
in-person
, 1:1 (one clinician to one participant)
No hour or unit limits; service frequency is based entirely on clinical need
Service intensity may increase or decrease based on the child's medical status and symptom progression
Minimum Qualifications:
Applicants must meet all of the following:
Minimum one year of experience working with medically fragile children or youth
, preferably in palliative care, pain management, or complex medical care
New York State licensure as one of the following:

Pediatrician or Family Medicine Physician
, board certified in Pediatrics or Family Medicine

Nurse Practitioner (NP)
licensed in NYS (Pain and Symptom Management)

Registered Nurse (RN)
licensed in NYS, under direct supervision of a board-certified pediatrician or physician
Strong assessment and clinical decision-making skills
Ability to communicate clearly and compassionately with children and caregivers
Ideal Candidate:

Skilled in pain management, symptom monitoring, and caregiver education
Comfortable working independently in home/community settings
Excellent at documentation, communication, and collaboration with care teams
Able to remain grounded, empathetic, and composed during challenging clinical situations
