Posting Summary
Rutgers, The State University of New Jersey, is seeking a Patient Representative for the Department of Medicine at the New Jersey Medical School.
Responsible for case management, the Patient Representative will assist the clinical and medical case management team with everyday duties; care coordination, advocacy, referral management, data entry, and to identify and remove barriers to medical care, to ensure adherence to the treatment plan. The CMA will work as a part of an interdisciplinary team of physicians, nurses, social workers, and certified medical assistants. The CMA will have varying responsibilities depending on the needs of the clinic.
Among the key duties of this position are the following:Identify upcoming and/or outstanding grant health maintenance required screenings.
Initiate annual financial eligibility screen for patients.
Assist with completion and management of forms (i.e., medication, emergency financial assistance, disability, medical transportation, medical assistive devices).
Greet patients courteously and updates their information or refer them to the appropriate staff.
Use a trauma informed care approach.
Create clear and concise documentation of services arranged as evidenced through chart review; documentation includes referrals coordination, follow-up, and completion of linkage to services.
Document all encounters accurately in the electronic medical record (EMR) within required timeframes.

Qualifications
Minimum Education and Experience
Requires a Bachelor's degree in Public Health, Social Work, or other health-related field plus two years of clinical or case management experience required; preferably working with HIV population.
Equivalent education, experience, and/or training may be substituted for the educational requirements.
