When a patient requires care from different facilities and/or practitioners they need a Patient Nurse Navigator to identify needs and remove any barriers to timely, appropriate care. Our Nurse Navigators guide the patient from diagnosis through treatment and act as the central point of contact for the patient and the entire medical team. It's a job that requires a strong communicator with excellent organization skills.

Patient Guidance

: The OB Navigator assists expectant mothers by providing prenatal education, resources, and emotional support throughout their pregnancy journey.

Care Coordination

: They coordinate care between healthcare providers, ensuring that patients receive timely and appropriate services. This includes scheduling appointments and facilitating communication among the healthcare team.

Advocacy

: Acting as a patient advocate, the OB Navigator helps patients navigate the healthcare system, addressing concerns and connecting them with necessary resources, such as community support services.

Education

: They provide information on prenatal care, childbirth preparation, and postpartum support, helping patients make informed decisions about their care. Assess patients for psychosocial distress and referral to a medical social worker or counselor if support that is more intensive is required. Patient & physician Liaison Answers questions as appropriate and serves as liaison to the physician for additional questions and concerns. Addresses tangible barriers to care such as finances, housing, transportation, childcare, prescription costs, and much more. Refers patients to the appropriate contact, such as a social worker, for support needed. Ensures any health information provided is accessible, such as securing documents printed in the patient's own language or arranging for certified medical interpretation, either in-person or by phone/videoconference. A member of an interdisciplinary team to assess, secure, and coordinate appropriate services through the continuum of care. Empowers patients, families, and caregivers to assume as much responsibility for their care as possible, within the constraints of age, illness, and other factors. Supports the timely scheduling of appointments, diagnostic testing, and procedures. Educates patients about their pregnancy, prescribed treatment, and side effects of treatment and how to manage them.

QUALIFICATIONS



Preferred Bachelor's Degree 1-year related experience Preferred Prefer Obstetrics experience Registered Nurse (RN) Basic Life Support Program (BLS) - American Heart Association Unit Specific Certifications - Franciscan Alliance
