As we transition to a membership-based model, we are expanding our team and seeking a Patient Navigator who is passionate about helping patients commit to improving their health while driving clinic growth.

Position Overview:

The Patient Navigator is the dedicated point of contact for new and existing patients - supporting them in understanding care plans, enrolling in memberships, scheduling appointments, and staying on track with their wellness goals.

This role exists to create a smooth, seamless experience for patients and to reduce overwhelm for our clinical staff by owning the scheduling, follow-up, and membership enrollment processes from start to finish.

The right person will not only guide patients but also drive revenue through successful membership and package sales, earning commission in addition to a competitive base hourly rate.

Responsibilities



Guide patients through understanding membership options, care plans, and package offerings. Help patients select the membership or services that best support their goals. Handle all scheduling needs for new and established patients, ensuring care plans are executed smoothly. Provide consistent follow-up to keep patients engaged, address concerns, and remove barriers (scheduling, financial questions, etc.) Reduce workload and overwhelm on front office and clinical staff by managing the administrative and logistical flow of patient care plans. Exceed monthly membership sales goals with professionalism and empathy. Create an encouraging, supportive patient experience that inspires commitment to long-term health. Act as the primary point of contact for patients, guiding them through the healthcare process from intake to discharge. Assist patients in scheduling appointments, understanding treatment plans, and helping with patients financials. Coordinate with multidisciplinary teams including Nurse Practitioners and Registered Nurses, Medical Technicians, and Administrative staff. Facilitate communication between patients and healthcare providers while maintaining strict adherence to HIPAA regulations for patient privacy and confidentiality. Provide education on medical terminology, discharge procedures, and follow-up care to promote patient understanding and engagement.

Skills



Excellent patient service skills with a compassionate approach that fosters trust and engagement. Strong organizational skills for managing multiple cases simultaneously while maintaining accuracy in documentation. Ability to navigate electronic health records (EHR) systems efficiently and securely handle medical records. Knowledge of HIPAA regulations ensures the confidentiality and privacy of all patient information is upheld at all times. Effective communication skills for liaising between patients, families and healthcare teams. Qualifications



Proven sales experience required. Healthcare, health insurance, medical device, or wellness sales preferred but not required. Associate's or Bachelor's degree preferred. Warm, caring, and persuasive personality - someone who can influence without pressure. Exceptional communication and relationship-building skills. Highly organized, proactive, and able to manage multiple patients and care plans simultaneously. Ability to stay calm and solution-oriented when patients feel overwhelmed. Passion for health, wellness, and helping people achieve better outcomes.

Compensation & Benefits:



Base hourly rate:

$22-$23/hour (commensurate with experience).

Commission:

Percentage per membership sold. High earning potential for strong performers.
