Responsibilities:
Collaborate with other professional disciplines to ensure effective and efficient patient care delivery and the achievement of desired patient outcomes Evaluate patient's progress toward attainment of outcomes Assist with the plan of care, evaluate, modify and coordinate care, and communicate well with other disciplines Maintain confidentiality with all information Perform any and all other duties as assigned

Qualifications:

Job Requirements:High school diploma or GED BLS certification must be obtained within 14 days of hire or transfer into the role and prior to providing direct patient care.

Job Preferred Requirements:One year, preferred. * Oklahoma Certified Nursing Assistant or Patient Care Technician, preferred.
