Primary Accountability:

The Patient Care Tech provides patient care as assigned, unit organization support, receptionist and clerical support to our Acute Care Department. The PCT is responsible for carrying out all aspects of the job, using problem-solving skills, strong interpersonal skills, and good judgement. In order to complement our facility's patient care delivery model, the PCT will support the team by anticipating and responding to team requests and patient needs.

Major Duties:

Receives patient assignments and institutes patient care for assigned patients. Introduces self to patients and family, and updates white board. Follows written policies and procedures as established by nursing administration in performing duties that relate to patient care and documentation regarding all aspects of patient care. Ensures that patient rooms are ready for admission and instructs patient and/or family how to utilize bed controls and call light system. Assists all patients with personal hygiene including ADL's, showers, perineal and oral care, and documents in the Electronic Medical Record (EMR). Changes bed linens and makes beds daily. Monitors and documents bowel movements, maintaining accurate intake and output in EMR. Assists patients in preparing for procedures. Assists with lifting, turning, moving, positioning, and transporting patients in and out of bed, chairs, wheelchairs, lifts, and documents in EMR. Assists RN's with duties in OB and ER as needed. Utilizes down time to stock, organize and clean department. Empties trash cans and wipes down room after OP dismissal. Demonstrates proper positioning and ambulation of patients. Takes vitals and patient's weight as instructed, and documents in EMR. Serves patient's meals, assisting with feeding and recording intake when necessary, and documents in EMR. Provides nourishments/snacks to patients in a timely and courteous manner. Reports all concerns or changes in patient's condition to the RN immediately. Cleans and disinfects patient care equipment in a timely manner. Tends to patients' physical, emotional, and spiritual needs. Provides a high level of customer service to patients, families, visitors and employees to promote and support the highest level of patient experience. Communicates effectively via a variety or modalities with patients, visitors, and facility staff; plays an integral role in communication across the continuum of care. Assists with inputting data for the department's quality measures. Requisitions supplies designated by nursing staff and performs housekeeping duties as necessary. Assists with patient admissions, transfers and discharges. Copies information, records, and reports from Acute Care charts to Certified Swing Bed charts. Assembles and maintains patient medical records and other documents related to patient care. Verifies that medical records contain complete and accurate patient demographic information, patient identification and registration in the medical records, including appropriate level of care. Assists with ensuring the attending provider signs progress notes, provider orders, etc., in a timely manner and in accordance with current regulations. Ensures that patient records and charges are complete and prepared for Health Information Management's (HIM) review. Reports to the Charge Nurse any discrepancies found in transcribing provider's orders, diet orders/changes, charting, etc. Keeps nurses station in order and organized with necessary paperwork, office supplies, forms, etc. Quickly and courteously acknowledges requests, responds to call lights, demonstrates proper phone etiquette, and fosters communication within the department.

Qualifications:

None required, on the job training provided - CNA Certificate preferred. BLS Certified within 60 days of hire.

Physical Demands:

Eye/Hand, Eye/Foot, Hand/Foot Coordination. Normal eyesight, with or without corrective lenses. Normal hearing, with or without hearing aids. The ability to bend, stoop and kneel. Full range of body motion including handling and lifting patients The ability to exert 50 to 100 pounds of force occasionally, and/or 25 to 50 pounds of force frequently, and/or 10 to 20 pounds of force constantly.

Work Environment:

Well-lit work area. Close visual work. Subject to frequent interruptions. May be subjected to infectious diseases, substances, contaminated articles including potentials exposures to the AIDS and Hepatitis B Viruses. May be subject to hostile and emotionally upset patients, family members, personnel, and visitors. Work with people under all conditions.
