Summary

As a Patient Care Coordinator at Advanced Care Home Medical, you will play a vital role in managing patient care and ensuring seamless communication between patients, families, and healthcare providers. This position is essential for maintaining high standards of patient satisfaction and effective service delivery.

Responsibilities

Coordinate patient care activities and ensure timely communication with patients and families. Manage patient scheduling and follow-up appointments to optimize care delivery. Assist in the preparation of medical documentation and maintain accurate patient records. Serve as a liaison between patients, families, and healthcare professionals to address concerns and inquiries. Provide administrative support to the medical team as needed, including handling phone calls and correspondence. Monitor patient progress and report any changes or issues to the healthcare team.

Requirements

Strong understanding of medical terminology and healthcare practices. Experience in medical administrative support or similar roles is preferred. Excellent organizational skills with the ability to manage multiple tasks effectively. Strong communication skills, both verbal and written, for effective interaction with patients and staff. Familiarity with office procedures in a medical setting is a plus.
