Job Title:

Care Coordinator for Senior Services

Location:

Onsite at various skilled nursing and long-term care facilities. Occasional travel between facilities in one workday required.

Schedule:

Part-Time to Full-Time, generally Monday to Friday, 8 AM to 5 PM, with a break for lunch

Compensation and Benefits:

Starting wage of $22-43 per hour (with profit sharing / bonuses). Substantial weekly incentives offered for new enrollments. Opportunity to transition to full-time with comprehensive benefits, including healthcare and unlimited PTO.

, you will play a crucial role in connecting our senior residents with the telehealth services that enhance their daily lives. This position requires a blend of technical savvy, excellent communication skills, and a compassionate approach to elder care.

Key Responsibilities:

Facilitate seamless telehealth connections for residents using tablets, headsets, and other essential equipment. Act as a liaison between residents and our clinical team, providing updates on connection issues, resident moods, and appointment scheduling. Maintain and troubleshoot technology setups to ensure reliable service during appointments. Develop positive relationships with residents and facility staff to create a supportive environment. Proactively identify and resolve issues to enhance the effectiveness of our telehealth solutions. Enroll new clients into the telehealth program, guiding them through the registration process and explaining the necessary steps. Inform potential clients about Empower Nation's services and offerings, highlighting the benefits and features of our telehealth solutions.

Requirements:

High school diploma or GED; additional training in healthcare or technology preferred. Strong familiarity with both Android and iOS platforms, and a knack for troubleshooting. Strong interpersonal skills to build rapport with a diverse clientele. Reliable transportation and willingness to travel locally on a daily basis. Flexibility to adjust hours as needed to meet residents' and facility requirements.

Pay: $22.00 - $43.00 per hour
