Part Time Testing Coordinator

Job Description

This is a part-time, grant-funded position with an anticipated duration of approximately eight (8) months.

Primary Objective



Reporting to the Director of Prevention Services, the Linkage to Care Coordinator serves as the coordinator for HIV Services, responsible for navigating activities related to HIV testing, linkage to HIV medical care, retention, and the re-engagement of persons living with HIV into medical care, treatment, and support services. The primary objective is to ensure timely access to HIV care and support services for individuals diagnosed with HIV.

Essential Functions



Implement ARTAS Strategy and Linkage Services:



Implement the Antiretroviral Treatment and Access to Services (ARTAS) strategy for individuals newly diagnosed with HIV or lost to care. Provide linkage to HIV care, treatment, and supportive services for all HIV positive individuals. Establish a Test-Link-Care Network with providers and community-based organizations offering HIV testing, counseling, or linkage services and referrals. Ensure all clients newly diagnosed with HIV or previously positive but not in care are linked to HIV care, partner services, and screenings for STDs, Hepatitis, and TB.

Rapid HIV Testing Coordination and Outreach:



Coordinate and implement Rapid HIV Social and Sexual Network Testing targeting MSM of Color. Develop outreach and testing calendar activities. Conduct Rapid HIV testing at traditional and non-traditional settings.

Staff Training and Program Collaboration:



Train staff and program participants on Social and Sexual Network recruitment. Lead program collaboration with agency subcontractors. Compliance and Record-Keeping: Adhere to HIPAA statutes and confidentiality policies. Assist in keeping up-to-date records on all patients and enter data into internal tracking systems.

Administrative Support and Reporting:



Assist with the daily functioning of the program and other general office duties. Attend meetings, conferences, trainings, and outreach events as needed. Complete monthly reports.

Referral Coordination and Patient Education:



Conduct referrals as needed based on assessment. Coordinate all aspects of HIV testing, including transportation, referral, and scheduling. Deliver patient education, counseling, and interventions to improve HIV knowledge and reduce transmission risk, including education on PrEP and PEP.

Documentation and Program Integration:



Organize and maintain complete, accurate records and file reports in a timely manner. Support the integration of programs into medical care.

Collaboration with Medical Providers and Flexibility:



Work with medical providers to determine health priorities and develop appropriate care management plans. May require evening and/or weekend hours to fulfill duties.

Travel Requirements:



Periodic travel may be necessary.

Knowledge, Skills, and Abilities



General knowledge of HIV/AIDS care processes, Ryan White eligibility requirements, and community resources. Strong communication, documentation, and interpersonal skills. Proficiency in MS Office and office productivity software. Timely problem-solving and efficient project management in a deadline-driven environment. Excellent written and verbal communication, critical thinking, and organizational skills. Dedication to the Center's mission, LGBTQ and MSM community knowledge, including the LGBTQ Ballroom community. Cultural competence and creative problem-solving. Team player with strong relational skills and the ability to work flexible shifts, including nights and weekends.

Education and Experience



Bachelor's degree in a related field (e.g., public health, social work, psychology) preferred.

Certification in HIV/AIDS & STI Prevention Counseling, Testing and Referral required.

3+ years of Rapid Testing Experience required.

Physical and Mental Requirements



Physical Demands:



Ability to use a computer for up to 8 hours a day. Flexibility to work in office, clinic, and community outreach settings. Local travel and occasional overnight training/travel may be required.

Mental Demands:



Ability to problem solve. Ability to work under pressure or in stressful situations. Ability to make quick decisions. Ability to multi-task Ability to use discretion and sensitivity.
