Part-Time Social Worker

What You'll Do:

Provide emotional support and guidance to residents and families



Assist with care planning, assessments, and discharge coordination



Work with the interdisciplinary team to ensure residents' needs are fully met



Help residents access resources and community services



Maintain accurate documentation and support smooth communication across departments

What You Bring:

Valid CT Social Work license (or eligibility)



Strong communication + problem-solving skills



Empathy, patience, and solid follow-through



Experience in a long-term care or healthcare setting is a major plus
