The Role:
Superior Hospice is seeking a Social Worker to provide emotional support and advocacy to patients and families across the Twin Cities. You'll serve as a steady, compassionate guide through end-of-life care, working with an interdisciplinary team that values purpose, presence, and partnership.
Key Responsibilities:
Complete psychosocial assessments and develop care plans Provide grief counseling, therapeutic support, and crisis intervention Help families navigate end-of-life decision-making and community resources Collaborate closely with RNs, chaplains, and physicians Document visits and interactions with accuracy and empathy
You'll Be a Great Fit If You:
Hold a
BSW or MSW from an accredited program
Haveprevious hospice or palliative care experience
Are an excellent communicator with a calming, supportive presence Know how to navigate both emotional complexity and regulatory compliance Have a passion for service and a deep respect for each unique end-of-life journeyWe'd love to meet you.
