The

Partner Service Professional

plays a vital role in supporting

Elara Caring

's mission by managing the referral process and ensuring seamless communication across teams. This position is responsible for converting referrals into active services, entering and maintaining accurate data, answering inquiries, and sharing information about the agency. The Partner Service Professional also coordinates referrals, supports daily operations, and fosters effective communication between Clinical Supervisors and the agency to help secure and manage patient care needs.

What You'll Need

Associate's degree preferred Minimum of 1 year of experience in a healthcare setting (required) Strongknowledge of medical terminology Excellent communication and interpersonal skills Ability to complete daily tasks accurately and efficiently Comfort working with patients who may be ill, disabled, or at end-of-life Capacity to thrive in a fast-paced, high-stress environment Physical ability to sit, stand, bend, lift, and move intermittently Receiving call Center experience is highly preferred Will need to live in a State where there is an Elara Caring Branch

A Day in the Life as a Partner Service Professional

No two days are exactly the same, but here's what you can expect:

Stay engaged in a

fast-paced, high-energy environment

where you'll handle about

60-65 calls each day

. Your strong communication skills and multitasking ability will keep things moving smoothly. Balance inquiries and referrals across multiple service lines, keeping your day dynamic and rewarding. Help patients and families by setting up services quickly and seamlessly when new referrals come in. Answer questions about Elara Caring's services and connect people with helpful community resources. Work closely with our Clinical Supervisors and team members to keep communication flowing and patient care on track. Maintain accurate, confidential records while ensuring all referrals are properly documented and tracked. Provide excellent customer service to both patients and partner organizations. Support Elara Caring's mission by ensuring every interaction reflects compassion, respect, and quality care. Jump in to assist with special projects or other responsibilities that keep our team running strong.
