Requirements

Current Michigan License in good standing Empathetic approach to patients and caregivers Participate in field visits daily, weekly office meetings as indicated Ability to adapt to patient's status and needs on a full time on call basis

Job Responsibilities:

Initial and on-going assessment of patient's physical, functional, psychosocial and environmental needs Implement the individualized plan of care and revisions as needed Document all patient/family visits, telephone contacts and referral actions timely, accurately and concisely in the patient record. Supervision of paraprofessionals providing services to patient Maintain high level of customer satisfaction with our clients, families and caregivers
