At PSCMH, we specialize in delivering compassionate
Palliative Care - Counseling and Support Services
under the Home and Community-Based Services (HCBS) Waiver. Our mission is to support children and youth with chronic medical conditions or life-threatening illnesses--and to help their families cope with the emotional, psychological, and practical challenges that come with these conditions. Services may begin at any point after diagnosis and may continue for up to six months after the participant's passing.
As a Counseling & Support Services Provider, you will play a vital role in helping families navigate grief, uncertainty, and stress by offering structured, therapeutic interventions that are empathetic, culturally sensitive, and clinically sound.
Responsibilities:
Provide grief counseling and emotional support to the participant and/or their family
Help families understand the child's chronic or life-threatening condition and manage associated feelings and stress
Facilitate individual counseling sessions and, when appropriate, support groups
Assist families in processing grief, planning for the future, and improving emotional resilience
Deliver goal-oriented care aligned with the HCBS Service Plan and Plan of Care
Collaborate closely with the Care Manager and the participant's healthcare team
Adjust service intensity as the participant's condition or family needs evolve
Provide timely, accurate documentation of all services
Adhere to all compliance expectations of PSCMH's Children's HCBS program
, including Medicaid documentation, service delivery standards, and recordkeeping requirements
Service Delivery:
Services are delivered
in-person
, individually (1 provider to 1 participant/family)
May be provided directly to the participant, to the family with the participant present, or to the family alone
Typical utilization:
0-5 hours per month
, depending on needs
Family services may continue for up to six months after the participant's passing, per HCBS guidelines
Minimum Qualifications:
New York State licensure as one of the following:

LCSW

LMSW

Licensed Psychologist

LMHC
-
LCAT
Minimum one year of experience working with medically fragile children or youth
, preferably in palliative care or chronic illness support
Strong interpersonal skills and the ability to provide sensitive, family-centered support during grief, uncertainty, and emotional hardship
Ideal Candidate:
Warm, grounded, and able to offer compassionate support in highly sensitive situations
Skilled at guiding families through grief, coping, and emotional processing
Comfortable collaborating with cross-disciplinary care teams
Detail-oriented with excellent clinical documentation skills
Emotionally resilient and able to maintain healthy professional boundaries
Job Type: Part-time
Pay: $75.00 per hour
