About PACE Program
The Midland Care PACE program is a specialized initiative in Kansas offering comprehensive medical, social and long-term care services to individuals age 55 and older who are eligible for nursing-home-level care but wish to live safely in their community. Through its centers in Topeka, Lawrence, Emporia and Kansas City, the program provides coordinated care--managed by an interdisciplinary team--that includes primary care, therapy, home-care support, transportation and day-center services. By helping participants remain in their homes rather than moving into institutional settings, Midland Care PACE aims to enhance quality of life, maintain independence and reduce hospitalizations and avoidable nursing-home admissions.
Position Summary
The primary focus of this position is to support IDT activities, scheduling appointments, data entry and maintenance of the Electronic Documentation System and files. This position is supervised as designated on the agency organizational chart.
Essential Duties & Responsibilities
Service
Maintains schedule information for clinical staff designated on the team.
Schedules participant clinical appointments and coordinates appointments with transportation and clinical
staff.
Prepares correspondence for team as needed.
Finance
Prepares faxes for various organizations to help facilitate the coordination of care; ie: pharmacies, coroners,
DME providers, and/or other contracting providers involved in the care.
Provides clerical support to clinical committees as assigned including agenda preparation, minutes, and notice
of meetings.
Documents interdisciplinary team meetings and other clinical documentation in the medical record as directed.
Enters data for claims processing.
Quality
Gathers data and generates reports specific to the team as directed.
Provides reports, medication lists, or other patient pertinent data to the clinical team as requested.
This may include but not limited to photocopying or printing documents for nursing facilities, third party
payers, etc.
Providing the clinical team with the appropriate information to facilitate the care to the client.
Files all original paperwork in clinical charts, and thins the charts when necessary.
Sends documents, satisfaction surveys, etc. by mail or facsimile as required.
Checks documentation for completion of signatures, titles, dates and required fields.
Assembles clinical documentation in "hard copy" charts and disassembles non-active patient charts.
Answers the telephone as needed for back-up support to receptionist.
People
Answers phone calls related to clients that are on the team and relays that information to the team.
Provides support to the clinical staff regarding medical record issues including technology issues.
Provides summaries of IDT meetings to the attending physicians.
Assists the team with faxing, mailing, or assimilating information regarding applications for Medicaid or other
services that may be identified as needed as part of the care being provided.
Provide support to team members in the field.
Facilitate the ordering and pick up of home medical equipment.
Growth
Attends staff and departmental meetings as required.
Serves as the recorder for performance improvement groups focusing on clinical issues.
Understands and demonstrates compliance with Midland policies and procedures.
