OUTREACH WORKERS (PT)
Monday to Friday 8:30 am to 2:30 pm
OR
Monday to Thursday 7:00 am to 2:00 pm


SUMMARY

Screens and assesses elderly candidates\xe2\x80\x99 eligibility for participation in homebound meal program by performing the following duties.


ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Educates the community on various homebound meal outreach program and the importance of utilizing the program
  • Distributes educational materials in the community
  • Visits homes to determine client needs, develops plan to meet needs
  • Identifies and monitors clients for homebound meal program
  • Completes required paperwork in compliance with state and program regulations
  • Administers initial nutritional screening in the form of questionnaire
  • Helps client obtain necessary resources
  • Refers clients with social and emotional problems to other community agencies for assistance
  • Refers clients to Director or Nutrition Services for nutrition counseling as needed
  • Documents client cases including assessment and reassessments and maintains records on each client
  • Prepares reports as required by various federal, state, and local programs
  • Helps organize and participate in fundraisers benefitting the program
  • Other duties may be assigned

EDUCATION/EXPERIENCE

Must have high school diploma and one to two years related experience and/or training.


LANGUAGE ABILITY
  • Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations
  • Ability to write reports, business correspondence, and procedure manuals
  • Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public



MATH ABILITY
  • Ability to calculate figures and amounts such as discounts, interest, commissions, proportions, percentages, area, circumference, and volume
  • Ability to apply concepts of basic algebra and geometry



REASONING ABILITY
  • Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists
  • Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form



COMPUTER SKILLS
  • Microsoft Word and Excel software
  • Ability to learn new software - Serv Tracker



CERTIFICATES AND LICENSES

  • Valid driver\xe2\x80\x99s license required
