The Outreach Behavioral Health Youth Case Manager coordinates the overall care coordination plan for youth referred to the Outreach Behavioral Health Team. This position is responsible for identifying treatment needs; developing and monitoring treatment plans and progress; working with families and making referrals to necessary interdisciplinary providers and community partners. Key responsibilities involve acting as an advocate, connecting youth with community resources, providing direct support, and maintaining accurate documentation.
Care Coordination will include assessing a young person's needs, developing, and implementing a case plan, and coordinating services to help them achieve goals related to education, housing, and social or emotional well-being. Services might include collaboration with existing services available through Island County, helping individuals with housing, benefits, mental health, medical care, substance use disorder treatment, employment and referrals to advocacy groups, schools, community liaisons and other needs as identified through assessments.

Job Duties

Provides community outreach and early intervention case management services to youth who are at risk of a behavioral health crisis or in need of intensive care coordination.
Conducts intake and needs assessments to understand a youth's challenges and goals.
Developing individualized service plans in collaboration with the youth and family as the means for implementing appropriate services. Responsible for coordinating services being provided; arranging resources, advocating for needed services, intervening in crises, and making appropriate referrals.
Coordinates with other professionals, such as teachers, counselors, and social workers. Advocates for the youth in meetings with schools, courts, or other agencies. Connects youth and families to community resources, including housing, educational, and mental health services.
Develops and maintain a working relationship with community providers which may include crisis stabilization facilities, crisis responders, behavioral health treatment providers, health care providers, shelter providers, landlords, and other community programs which support youth.
Documents all contacts in a timely basis including face-to-face interviews, collateral and networking contacts, and correspondence. Maintains case records in accordance with standards and requirements. Prepares reports by collecting, analyzing, and summarizing treatment and results data.
Prepares case closure in a timely manner by reviewing and communicating care plans, orienting family members and other collateral contacts and providing resources. Provides follow up services when appropriate and necessary.
Communicates clients' progress by attending weekly interdisciplinary meetings and supervision in compliance with agency policies and procedures. Collaborates and works with other community agencies.
Updates job knowledge by participating in educational opportunities, reading professional publications, maintaining personal networks, and participating in professional organizations.
Follows policies, procedures, and protocols for this program as a member of the Outreach Behavioral Health Team.
Performs other duties as assigned by the supervisor which are consistent with the position.
Knowledge, Skills & Abilities

Strong knowledge and understanding of adolescent development and ability to collaborate well with youth and families. Capable of relating well to individuals from diverse backgrounds, cultures, education levels and varied socio-economic backgrounds.
Capability and willingness to work independently under general supervision in a self-directed manner and as part of a team. Possess good time management, organizational, and problem-solving skills. Ability to quickly grasp new techniques, deal with rapid changes in events, remain calm in stressful situations, relate to various individuals in a calm, professional manner.
Ability to communicate clearly and effectively, both verbally and in writing, with people from a variety of cultural, economic, and ethnic backgrounds; individuals experiencing disabilities; and individuals for whom English is not their first language. Able to identify community resources and services for families and coordinate provision of services; to establish effective working relationships with internal county staff as well as with community partner organizations.
Understands the need for maintaining confidentiality of information. Familiarity with client record documentation requirements, collaborative care plan development and implementation.
Ability to accurately complete data entry and clearly transmit information through email. Have a working knowledge of Microsoft Excel, Outlook, and Word and the capacity to work effectively with different software applications.
Education, Experience & Certifications

Bachelor's degree in Social Work, Psychology, Sociology, Human Services, Human Development or related field required
Two or more years of experience in providing youth care coordination or case management or working with at-risk youth required
Prior relevant experience and training may be substituted for years of experience
Valid Driver's License.
