Job Description

I. Qualifications



Skilled Nurse from an Accredited program currently licensed and in good standing with the New Jersey State Board of Nursing. 1-2 years' experience in nursing in an acute care setting or 1-year experience in home health, hospice or comparable experience is preferred. Desire to learn and demonstration of aptitude for the hospice philosophy and comfort care required. HomeCare HomeBase experience preferred.

Must be willing to travel within 60 miles from office location.

II. Responsibilities



An on-call hospice nurse is a registered nurse (RN) who is available outside of regular business hours to respond to urgent calls from hospice patients and their families, providing immediate assessment, symptom management, and care as needed, including potential home visits, while coordinating with the primary hospice team to ensure continuous care during after-hours situations.

A list of duties include:

Assessing patient concerns and determining the urgency of the situation through phone calls from patients and families. Performing home visits as required to assess patient conditions, administer medications, manage symptoms, and provide comfort care. Admitting patients to hospice when necessary. Pronouncing death of a hospice patient when necessary. Communicating with the primary hospice care team, including physicians, social workers, and other nurses to ensure coordinated care. Providing emotional support and education to patients and their families during critical situations. Develops and re-evaluates the patient/family care plan in conjunction with IDG to meet needs and maintain continuity of care. Performs specific nursing procedures as need (e.g., treatments, management of symptoms) following doctor's orders. Maintains patient records as required by Hospice. Follows the policies and procedures of the Hospice. Observes confidentiality and safegaurd all patient-related information in compliance with HIPAA regulations. III. Orientation and Training



Lilycare of New Hampshire will provide orientation and training, as necessary, to assist the On-Call Nurse in performing the responsibilities listed above. Training may be in-house or outside seminars. Evidence of orientation and training will become part of the personnel file.

IV. On Call Schedule



The schedule for this position will be Monday-Thursday from 4:30pm-8:30am each day, as well as Friday 4:30pm-Monday 8:00am on a 7 day on, 7 day off rotation.

Job Type: Full-time

Pay: $90,000.00 - $95,000.00 per year
