Job Description

The LTC Unit Manager provides leadership and clinical oversight for a designated nursing unit in a long-term care facility. This role ensures the delivery of high-quality, resident-centered care while maintaining compliance with CMS regulations, Massachusetts Department of Public Health (DPH) requirements, and facility policies. The Unit Manager supervises nursing staff, coordinates resident care plans, and supports a culture of safety, compassion, and continuous quality improvement.

Key Responsibilities

Leadership & Supervision



Direct, coach, and evaluate RNs, LPNs, and CNAs assigned to the unit; oversee daily staffing and scheduling. Provide leadership during surveys, audits, and inspections; ensure readiness for CMS and DPH compliance. Promote an environment of teamwork, respect, and resident-centered care.

Clinical Care Oversight



Assess residents' health status, review physician orders, and monitor nursing interventions. Coordinate interdisciplinary care plans and ensure timely documentation in the EMR (e.g., PCC). Oversee infection control practices and adherence to standard precautions.

Quality & Regulatory Compliance



Implement QAPI initiatives and monitor key quality indicators (falls, pressure injuries, infection rates). Ensure accurate completion of MDS assessments and care plan meetings in collaboration with the MDS coordinator. Maintain compliance with 105 CMR and all federal, state, and local regulations.

Education & Staff Development



Provide in-service training, competency checks, and mentoring to nursing staff. Identify staff development needs and recommend continuing education opportunities.

Communication & Collaboration



Serve as primary contact for families, physicians, and ancillary providers regarding resident care. Participate in daily management meetings and contribute to emergency preparedness planning.

Qualifications

Licensure:

Current Massachusetts Registered Nurse (RN) license in good standing.

Experience:

Minimum 2-3 years of clinical experience in long-term care or subacute rehabilitation; prior leadership or charge nurse experience preferred.

Knowledge/Skills:



+ Familiarity with CMS regulations, MDS process, and DPH survey standards.
+ Strong leadership, delegation, and problem-solving abilities.
+ Excellent interpersonal and communication skills.
+ Proficiency with electronic medical records.

Physical & Working Conditions


----------------------------------

Ability to stand, walk, and move intermittently during work shifts. Occasional lifting of up to 50 pounds and assisting with resident transfers. Exposure to infectious diseases and biohazard materials with appropriate PPE.

Schedule & Compensation


----------------------------

Full-time, typically Monday-Friday with occasional weekends or on-call duty. * Competitive salary commensurate with experience; comprehensive benefits package.
