Overview:

Precision Rehabilitation LLC is a leader in creating a dynamic experiences for patients in skilled rehab settings. Our success stems from working collaboratively with the onsite rehab team to help patients get back to what they love. We are currently hiring board certified nurse practitioners and physician assistants in the Sumter area. Together we can inspire our patients to achieve their highest functionality so they can return to their previous living arrangement confident in their abilities. This is a fulltime position Monday-Friday with no on call and no weekends.

Duties:

- Conduct diagnostic evaluations and develop treatment plans
- Assist rehab team with DME selection
- Collaborate with hospital medicine teams for comprehensive patient care
- Document in patient EMR

-Participate in care planning meetings

-Meet with patient and family for rehab setting goals

-Communicate with nursing team on medical issues

Requirements:

- Valid Nurse Practitioner license
- Proficiency in memory care practices
- Prior experience in hospital medicine or clinic settings
- Excellent communication and interpersonal skills

Pay: $105,000.00 - $115,342.10 per year
