What You'll Do

As a

Nurse Practitioner

with our Hospice Care team, you will play a vital role in ensuring patients continue to receive compassionate, appropriate care during their hospice journey. In this flexible, independent position, you will:

Conduct

face-to-face evaluations

with hospice patients to assess clinical condition and prognosis. Collaborate with the

interdisciplinary team

to document patient status and support recertification decisions. Complete

timely, accurate documentation

in compliance with Medicare and Andwell standards. Communicate with empathy, providing education and support to patients and families. Maintain compliance with all

legal, regulatory, and organizational

standards of care.

What You'll Bring

Current, unrestricted

Nurse Practitioner (NP)

license in Maine. Certification as an

Adult, Geriatric, or Family Nurse Practitioner

preferred.

Hospice experience preferred

-- training provided for those new to hospice care and new grads encouraged to apply. Familiarity with

Medicare hospice guidelines

and documentation standards preferred. Excellent

communication, interpersonal, and assessment

skills. Ability to

work independently

and travel to patient locations.

Schedule & Compensation

Schedule:

Flexible; as needed based on patient needs and recertification timelines.

Compensation:

$60.00 per hour, paid upon completion of documentation and encounter requirements.
