Summary

We are seeking a skilled Nurse Practitioner to join our team at Jay Medical Clinic. In this role, you will provide primary care in outpatient and senior living / nursing home setting , focusing on the health needs of our patients from teenagers to seniors. Your expertise will be vital in enhancing patient outcomes and delivering exceptional healthcare.

Responsibilities

Conduct thorough physical examinations and assess patient health status. Manage treatment plans for various conditions. Provide primary care services for adults and geriatric populations Collaborate with physicians and other healthcare professionals to ensure comprehensive patient care. Educate patients and their families on health management and preventive care strategies. Utilize electronic health records for accurate documentation and patient tracking.

Qualifications

Valid Nurse Practitioner license in the state of practice. Experience in outpatient settings with a focus on primary care. Strong skills in patient assessment and treatment Familiarity with ICD-10 coding for accurate documentation. Ability to provide compassionate care. Experience with electronic health record systems, preferably Eclinicalworks ( ECW), is a plus.
