Position description
Advanced Care at Home (ACH) is a care model that delivers innovative, comprehensive, and complex care to patients\xe2\x80\x94all from the comfort of patients\xe2\x80\x99 homes via an innovative technology platform. Through ACH, patients with conditions previously managed in the hospital setting can transition to their homes to receive compassionate, high-quality virtual and in-person care.

ACH is part of the Division of Hospital Internal Medicine at Mayo Clinic in Arizona.

The Nurse Practitioner/Physician Assistant (NP PA) works cooperatively with physicians, NP PAs, nurses, and other allied health personnel.

The NP PA will perform the following tasks within the hospital or the patient\xe2\x80\x99s home setting:

• Obtain a patient history
• Perform a physical examination
• Order and perform diagnostic and therapeutic procedures
• Formulate a diagnostic impression
• Develop and implement a treatment plan
• Monitor the effectiveness of therapeutic interventions
• Facilitate appropriate consultations and referrals
• Provide counseling and education to patients and their families

The NP PA will also:

• Maintaining prescribing and privileges in Arizona

100% of days worked will require the NP PA\xe2\x80\x99s presence on the hospital campus and travel to patients\xe2\x80\x99 homes.

Qualifications
Physician Assistant:
The PA must be a graduate of a PA program accredited by the Accreditation Review Commission on Education for the Physician Assistant, Inc. (ARC-PA). NCCPA certification is required, and the PA must be licensed to practice as PA in the State of Arizona.

Nurse Practitioner:
The NP must be a graduate of a NP program accredited by the American Nurses Credentialing Center (ANCC) and have completed an Acute Care certification. A Master\'s degree in Nursing is required. The NP must have a current Arizona Advanced Registered Nurse Practitioner (ARNP) license.

Additional qualifications
At least one year of hospital-based experience is required. The NP PA must be flexible and willing to work 12-hour shifts, weekends, and holidays. Experience providing care in patient\xe2\x80\x99s homes or via telehealth/remote monitoring is a preferred. The NP PA must have excellent verbal and written communication skills and possess strong computer skills.

License or certification

• Nurse Practitioner or Physician Assistant license to practice in the State of Arizona.
• BLS and ACLS are required.
• For Nurse Practitioners, Acute Care certification by the ANCC is required, and for Physician Assistants, NCCPA certification is required.
• Evidence of application for DEA certificate must be provided upon hire and obtained within 90 days of hire. Certification should be maintained thereafter unless waived by department/division.
• A valid driver\xe2\x80\x99s license and an acceptable driving record per an annual motor vehicle record check are required.
